Please contact our office to schedule an appointment.
Check in
Please arrive 15 minutes prior to your appointment, to complete paperwork.
Cancellation policies and fees
For cancellations without 24-hour notification, a $75 fee will be incurred.
Payments and fees
All co-pays and deductibles are due at the time of your appointment.
Form fees
School, camp and medication forms will be completed within 72 hours of receipt.
The fees are $25 for complex forms and $15 for simple forms.
Insurances
We accept most insurance. Please contact our office to verify.
